Eligibility
Applicants must meet specific academic and residency requirements to be considered. The program accepts high school seniors and currently enrolled students at accredited U.S. colleges and universities. Preference is given to applicants with a physical disability, but all eligible candidates meeting the criteria may apply. A minimum cumulative GPA is required.
Eligibility: U.S. citizen or Alien Registration Card holder; high school senior or enrolled in an accredited U.S. college/university; minimum cumulative GPA of 3.0; pursuing a degree in a medical-related field (examples include nursing, medicine, therapy, nutrition, laboratory science, dentistry, health law, etc.).

Submission Requirements
Submissions must be original, unpublished work about an act of kindness that inspired the applicant; the story need not be related to the applicant's medical studies. This scholarship is merit-based; statements of financial need do not affect selection. Acceptable formats include written entries, short videos, or images, each with specified limits and guidelines. Finalists are selected by judges and posted for a time-limited public voting period that factors into the final decision.
Submission Formats: Writing — up to 350 words (optional image); Video — up to 1.5 minutes in standard formats (.mov, .mp4, .m4v, .flv, .3gp, .avi, .wmv); Image — up to 8.5 x 11 with a short description.
Application Timeline: Finalists are notified after the submission deadline; entries for the 2026 award must be received by the winter deadline (received by midnight Eastern Standard Time). Voting is open for a limited period on the scholarship page.

Submission Rules
All submissions will be judged on content, creativity, and quality of work. Entries with grammatical or spelling errors or otherwise inappropriate content may be disqualified. AvaCare Medical reserves the right to publish submissions, including winners. Ensure your submission meets quality and content standards before sending.

How to Apply
There is no formal application form: submit your project, a high school or college transcript, and the requested applicant information by mail or email. Video submissions must be sent by email only. If mailing a written or image submission, keep a digital copy in case the work is posted online. Do not send additional documents such as letters of recommendation, resumes, or CVs.
